Healthcare Navigation

Direct, practical assistance connecting newcomers to family physicians, walk-in clinics, Community Health Centres (CHCs), and specialized care providers across Waterloo Regionβ€”ensuring timely access to essential medical care and emergency services.

Medical Appointments

Equipping newcomers with the confidence and tools to attend medical visits effectivelyβ€”including practical appointment preparation, interpretation navigation, advocacy support, and coordinated follow-up care within the Waterloo Region healthcare network.

How It Works

STEP 1

Connect with staff

A dedicated Community Initiatives team member connects directly with the client to understand their health priorities, personal well-being goals, and immediate settlement barriers in a culturally safe and welcoming environment.

STEP 2

Review needs

We collaborate with the client to evaluate their overall situation, key documentation, pressing health or settlement questions, and immediate well-being priorities to establish a clear, coordinated path forward.

STEP 3

Plan next steps

We work alongside the client to map out tailored next steps, identifying essential medical appointments, specialized partner referrals, wellness programs, and localized community resources across Waterloo Region to meet their well-being goals.

STEP 4

Follow up

Our team maintains continuous, responsive follow-up as health journeys evolve, appointments progress, and new community opportunities ariseβ€”ensuring seamless adjustments and sustained well-being across Waterloo Region.
